Patients have the right to access their medical records. Health Service Alliance makes it easy to request copies of your records securely.
To begin a medical records request, please follow the instructions below:
Important: this form must include a wet (handwritten) signature
Digital or typed signatures will not be accepted
Please scan, take a clear photo of the completed and signed form before emailing. You may also bring the form into any of our locations for processing.
Please allow 7-10 days for processing after your completed request has been received.
Requests may take longer if:
Addition authorizations is required
Records need to be retrieved from archieved systems
Information is incomplete
