If you sent us your contact details through this website or through one of our Facebook or Instagram forms, you can ask us to delete them. Here is how.

What this covers
This page covers information you gave us online: a coverage check, the questionnaire, a request to be called back, or a Facebook or Instagram form where you asked us to contact you. Our Facebook and Instagram tools only manage our own ads and receive the forms you choose to send us. They do not read your Facebook or Instagram account.
Your clinical records as a patient are governed separately by HIPAA and by our Notice of Privacy Practices, and have their own retention rules. Ask at your health center about those.
How to ask
Email info@hsa.care with the subject line "Delete my data", or call 877-335-2559, Monday to Friday, 9 AM to 5 PM, and tell our front desk you want your website or social media inquiry deleted. Include the name and phone number or email you used, and where you sent it (the website, Facebook, or Instagram) so we can find it. We may ask one or two questions to confirm it is really you.
What happens next
We confirm your request within 10 business days and delete the inquiry from our website records and from our Facebook and Instagram leads within 30 days, unless the law requires us to keep it. If you also want it removed from Facebook or Instagram itself, you can delete your own form responses in your Facebook settings under "Your information".
