The pattern is well documented
Across the United States, lower household income tracks with higher rates of diabetes, heart disease, obesity and untreated mental health conditions. The Centers for Disease Control and Prevention and the Kaiser Family Foundation both publish this in detail, and it holds across states and decades.
What the pattern is not is a story about individual discipline. It is a story about the conditions people are making decisions inside.
What is actually driving it
- Cost at the moment of care, which turns a checkup into something to postpone
- Time and transport, especially where a visit costs a shift's pay as well as a copay
- Food that is cheap, filling and close by, which is rarely the food a clinician would pick
- Housing that is crowded, unstable or unhealthy in ways a prescription cannot reach
- Chronic stress, which is a physical force on the body and not merely a mood

Why prevention is the first casualty

Does asking about the sliding fee affect anything else?
No. It is a discount program for our own charges. It is not a public benefit application and it is not reported anywhere.
Can you help me get covered rather than just discount the visit?
Yes, and we would rather do both. Our front desks help people apply for Medi-Cal and renew it at no cost, whether or not they are already patients.
